In Western Australia there are essentially 2 ways to change your name.

Changing your name by Registration


You will need supporting documents such as your birth certificate (and marriage certificate). There is a fee to pay. Ring the Attorney Generals Department first and find out what documents you need in order to perpare and register a name change.

Changing your name by marriage

There is no legal requirement to "officially" register a change of name to that of your spouse. You can simply assume the name of your spouse if you want to. You can choose to return to using your maiden without making it official. You just need to advise the relevant organisations or authorities that you wish to be known by your former name.

A woman who has been married more than once cannot use the name of a former spouse without making the change formal through Deed Poll or Licence.

Note! Changing the name of a child can only be done by Licence. Further information should be sought from the Registrar General's Office and/or a solicitor.
